Hall of Fame Nomination Form

Qualifications for candidacy to the Helias Catholic High School Hall of Fame:

The Candidate:
A. Graduated from high school at least 20 years prior to induction (REQUIRED)
B. Has demonstrated service and loyalty to Helias after graduation (REQUIRED)
C. Exhibits high moral standards to the credit of Helias Catholic High School
D. Must be or has been an exemplary member of their community and demonstrates the qualities of good character and citizenship
E. Has distinguished him/herself within his or her chosen field
F. Is generally a Helias graduate, but not necessarily
G. Was an above-average student in high school in academics, and/or in extracurricular activities

Criteria A and B are MANDATORY
